1. Current insurance cards - please bring both Major Medical and Vision Insurance.
2. Current and/or old glasses.
3. Current sunglasses.

If you are having a contact lens exam

1. Bring your contact lens box that has contact lens information for each eye.
2. Wear your contacts for at least 2 hours prior to your exam so we can assess if they fit properly.
